This is a fantastic opportunity to work with adults who have experienced mental health challenges, substance misuse, and complex needs, including individuals transitioning from residential rehabilitation. The Role
- You'll work across four supported accommodation sites in Westminster, providing person-centred support that helps residents build confidence, maintain their accommodation, and move towards greater independence.
- Deliver one-to-one support and regular key work sessions.
- Complete support plans, risk assessments, and reviews.
- Promote recovery, wellbeing, and independent living.
- Support residents to access healthcare, benefits, employment, training, and community services.
- Work closely with mental health teams, probation, substance misuse services, and other partner agencies.
- Maintain accurate records and contribute to positive outcomes for residents.
About You
- Experience supporting adults with mental health, homelessness, substance misuse, or complex needs.
- A person-centred and trauma-informed approach.
- Strong communication and organisational skills.
- Ability to work independently across multiple sites.
