Join Our Brent Community Paediatric Team as a Clinical Nurse Specialist. Are you an experienced and compassionate Registered Nurse looking to make a real difference to the lives of children and young people? We have an exciting opportunity for a Clinical Nurse Specialist to join our Brent Community Paediatric Team, supporting children and young people aged 0–19 years with special educational needs, additional needs and disabilities.

Working within a supportive multidisciplinary team, you'll provide specialist nursing assessment, care coordination and ongoing support from referral and diagnosis through to long-term medical and therapeutic care. You'll develop and implement personalised healthcare plans, work closely with families and partner agencies, and play a key role in improving outcomes for children with complex needs. This role also offers the opportunity to contribute to service development, provide clinical leadership and develop a specialist area of interest as your career progresses.

We're looking for a Registered Nurse (RN, RSCN or RGN) with significant experience working with children and young people within a multidisciplinary setting. A Specialist Community Practitioner qualification in Health Visiting, Children's Community Nursing or School Nursing is desirable.   • Triage referrals and provide families with information, advice, and support to access the most appropriate services.
  • Undertake comprehensive assessments of children and young people with complex disabilities and neurodevelopmental needs, including developmental assessments and detailed clinical histories.
  • Manage and coordinate specialist care, providing follow-up support through clinics, telephone consultations, and home visits where appropriate.
  • Contribute to Education, Health and Care Needs Assessments (EHCNAs) and produce high-quality health advice and reports for EHCPs.
  • Run nurse-led clinics and review children following medication changes or multidisciplinary reviews, escalating concerns when required.
  • Work collaboratively within multidisciplinary and multi-agency teams to plan and deliver effective, family-centred care.
  • Provide specialist advice, training, and support to families, carers, and professionals.
  • Support children and young people through key transition periods, including transition to adult services.
  • Use advanced clinical judgement to independently manage defined clinical pathways within professional scope of practice.
  • Contribute to service development and quality improvement, identifying innovative ways to enhance care delivery for children with disabilities and complex needs.
