To provide a specialist speech and language therapy service as part of the Community Neurological Rehabilitation Team for adults within the tri-borough area of Hammersmith & Fulham, Kensington & Chelsea, and Westminster. The post holder will work in collaboration with the community teams within CLCH, other health care professionals, statutory and voluntary agencies to deliver excellent standards of patient care.
The post holder will be based within one of the three MDT neurorehabilitation teams, and provide specialist assessment and treatment of clients with a wide range of acquired neurological conditions with swallowing and/or communication impairments. The post holder is also part of the Early Supported Discharge Team (ESD) for Stroke. This includes the opportunity to work as a service coordinator, triaging and managing ESD referrals from acute site partners.
The post holder will be an integral member of the community neuro-rehabilitation team based in the specified borough, acting as a resource to other medical, nursing, therapy, social services and voluntary agency staff. They will build and maintain key relationships with referrers to the service (including GPs, acute hospitals, bedded services, Care or Neuro navigators) to facilitate appropriate referrals through promoting understanding of the role of the speech and language therapist within the multi-disciplinary team (MDT).
The role covers a variety of settings including clients' homes, out-patient clinics, residential and community facilities. The post holder would be expected to provide advice, training and support to nursing home staff, students, therapy assistants and others.
