Central London Community Healthcare NHS Trust has been delivering West Hertfordshire Community Adult Services since October 2019. We continue to develop high-quality, patient-centred services in partnership with local communities and acute providers.

Exciting opportunities have arisen within our Neuro Rehabilitation Inpatient Unit in West Hertfordshire. We are seeking a Band 3 Health Care Assistant to join our supportive and well-established multidisciplinary team. Participating in a 7-day working rota, including weekend shifts and bank holidays.

This role offers the chance to work within a bedded rehabilitation unit, supporting adults with a range of physical disabilities and functional impairments resulting from neurological conditions. The unit provides specialist rehabilitation for adults referred predominantly from acute hospital settings, with additional referrals from community settings.

You will be part of a collaborative team committed to high standards of rehabilitation and care. We support learning, skill development, and service improvement, making this a rewarding opportunity for those looking to grow their experience in rehabilitation within a dynamic NHS setting.

The Health Care Assistant will support patients to work towards agreed treatment goals, with a strong focus on improving independence and quality of life. This role involves promoting daily living skills, supporting personal care, and maintaining strength and physical function.

You will manage patients and assist qualified staff in delivering individual and group treatment programmes within an agreed scope of practice. Working as part of a dedicated multidisciplinary team, you will contribute to nursing interventions and ensure clear handover of patient progress and any concerns to relevant ward or care home staff.

The role also includes supporting discharge processes, such as assisting with nursing discharge documentation and referral forms as well as managing stock take processes to ensure accurate inventory control.

We are looking for a motivated and compassionate individual who is committed to supporting patients to achieve their goals. You will be able to work independently when required, demonstrate excellent communication, organisation, and problem-solving skills, and manage your time effectively.
