Central London Community Healthcare (CLCH) NHS Trust provides a wide range of community-based services. The Hertfordshire Division is currently recruiting for two Band 7 Community Palliative Care Clinical Nurse Specialists, with one full-time and one part-time (0.8WTE) permanent role available.
The successful applicants will join a supportive, highly motivated multidisciplinary team working collaboratively to deliver high-quality specialist palliative care within the community. The Community Palliative Care Clinical Nurse Specialist (CNS) will be part of an established, integrated palliative care team supporting patients with various cancer and non-cancer diagnoses, including long-term conditions.
The role contributes to the wider cancer and palliative care agenda, helping to deliver a flexible, responsive service that reflects local population needs across Hertfordshire. As this is a community-based role, a full, valid UK driving licence and access to a car for business purposes are required (unless you have a disability as defined by the Equality Act 2010).
Key responsibilities include:
- Providing highly specialist palliative care advice and support to generalist nurses, care agencies, and other health professionals.
- Using advanced communication, clinical judgement, and decision-making skills within palliative care settings.
- Managing a caseload of patients with complex needs and ensuring appropriate specialist palliative care support.
- Triage messages from patients on the current caseload or after a new referral to offer clinical support, advice, and assessment.
