Hersham Surgery is seeking an enthusiastic and experienced Diabetes Practice Nurse to join our patient-centred team. The role involves providing high-quality diabetes care, including annual reviews, diabetic foot checks, blood pressure and cardiovascular risk assessment, lifestyle advice, medication support and ongoing management of Type 1 and Type 2 diabetes.
You will work closely with GPs, nursing colleagues and the wider multidisciplinary team to support patients in managing their condition.
