The post holder will be responsible for coordinating and navigating the patient across Intermediate Care Service pathways to ensure effective discharge planning of complex patients who are deemed to be medically optimised includingfacilitatingandinfluencingthedischargeor admission avoidanceprocess.
The post holder will be responsible for improvingpatient health outcomes and ensuring efficient bed utilisation and patient flow.
The Mental Health Nurse will need to be able to workautonomouslyand aspart of amulti-disciplinaryteampromoting patient centeredcare.
Within the Acute/Community the postholder will be assessing, planning, implementing and evaluating patient care demonstrating evidence based/best practice, considering own clinical confidence and competence within their sphere of mental health practice.
The post holder will need to demonstrate the ability to implement enhanced programmers and maintain high standards of clinical intervention to provide Nursing care for service users accessing the range of intermediate care services.

The post holder will be part of an integrated service inclusive of health and social care staff, working collaboratively with both primary and secondary care professionals.
The post holder shall act as an expert resource for the multidisciplinary team and work collaboratively with Care Home staff and management for all patients that are placed onto Pathway One, Two or Three and awaiting further health and social care assessments.
The post holder will review and assess patients that are Walsall residents or have a registered Walsall G.
P on IC S Pathways One, Two or Three with an identified Mental Health condition or need.
The post holder should be able to identify the key stages where the patient will require interventions that influence their care, this shall include, but not limited, to contributing to the implementation and monitoring of assessment tools to ensure optimum safety and care in relation to Mental Health conditions.
The post Holder shall empower staff in their personal and professional development to undertake a greater range of skills to modernise and improve patient care, including the uptake of training related to Mental Health conditions.
The post holder will be a source of specialist knowledge and expertise, providing support to the integrated team in their day-to-day management of patients who present with Mental Health conditions.
Excellent communication skills are essential for this role.

- Ensureresourcessuch as 1:1 care in a secondary care settingis usedinacosteffectiveandefficientmanner.
- To make initial response to complaints following the Trust’s complaints procedure in line with the Trust policy.
- To allocate workload to junior members of the team when appropriate and in the absence of the Team Manager.
- To line manage Band 3-5 where appropriate.
- Work with hospital and community managers to collect activity data that will inform and guide service delivery.
- Work with Team Manager to develop `Risk Management’ procedures that enhance continuityofpatientcareandminimisetherisksassociatedwithdischarge/transferof care to the primary setting and ongoing care in the community.
- Act in aprofessionalmanneralways whilston Walsall Healthcare NHS Trusts business and demonstrateexcellence in clinical practice, ensuring compliance to Walsall Healthcare NHS Trust policies and procedures.
- Participate and lead in multidisciplinary team meetings, decision support tool assessments, continuing health checklists, joint assessments with the community Mental Health team and best interests’ meetings where appropriate.
- Provide professional and clinical leadership, mentorship and clinical expertise to the staff caring for patients with Mental Health conditions within IC S.
- Provide leadership on the delivery of evidence based best practice for Mental Health conditions.
- To be responsible for the protection of individuals from abuse and harm in line with local safeguarding policies and procedures, ensuring the appropriate sharing of information
- To play a proactive role in quality and service improvement and work closely with the IC S team manager and the Mental Health matron and multi-disciplinary team will contribute to the monitoring and auditing of clinical standards of care of patients on the IC S pathway.
