To work as a skilled member of the nursing team within Transitional care providing a high standard of care for the neonate and family. To act as a role model and clinical expert practitioner in neonatal care. Ability to promote family centred care and establish good relationships with babies and their families. To communicate effectively at all levels within the multi-disciplinary team. Work with the Community Neonatal Outreach team to support families in preparation for discharge. To promote a progressive attitude to the continual improvement of patient care through research and evidence-based practice. To act as a named nurse for neonatal patient care on Transitional care to support families with a smooth discharge care pathway. Ensure safe provision of care through Transitional care, linking in with the Neonatal Unit and Delivery Suite being aware of upcoming activity. Deliver high standards of compassionate, dignified care, managing time accurately and efficiently in cooperation with other staff members, tasks and resources. Undertake training, assessment and facilitation of peers and staff. Work within the boundaries of existing competence, adhering to local and national protocols / standard operating procedures.

Job Purpose: To provide high-quality, family-centred neonatal care within the Transitional Care setting, supporting babies who require additional monitoring, treatment, or feeding support while remaining with their mothers where appropriate. Working with families and the multidisciplinary team, the post holder will deliver safe, effective, evidence-based care and support positive outcomes for infants and their families.

Key Responsibilities:

  • Deliver safe, compassionate, evidence-based care to infants in Transitional Care.
  • Maintain clinical competencies, including the safe administration of neonatal medications.
  • Recognise deterioration and elevate concerns appropriately.
  • Ensure the safe use and maintenance of clinical equipment.
  • Support, educate, and empower parents in caring for their baby.
  • Promote family-centred care and advocate for babies and families.
  • Plan and coordinate discharge from admission, liaising with relevant services.
  • Communicate effectively with families and the multidisciplinary team.
  • Maintain accurate records and use PAS in line with Trust policies.
  • Comply with safeguarding, infection prevention, health and safety, and governance requirements.
  • Participate in audit, quality improvement, training, and service development activities.
  • Maintain NMC registration and undertake ongoing professional development.
