Senior Physical Health Check Coordinator
About you
You will:
- Speak to individuals to who are on the SMI (Severe Mental Illness) register, describe the benefits and what happens at a Physical Health Check and enable their appointment.
- Develop a rapport with individuals and identify barriers to accessing services and support to attend local services and interventions that could improve their physical health.
- Contact individuals after their Physical Health Check, discuss the results and any recommendations for improving their physical health.
- Work autonomously across the Locality to implement the service and drive its success.
- Build relationships with stakeholders, particularly Primary Care Networks, increase awareness of the service and codesign processes across the Locality.
- Promote recovery, inclusion, and equity in everything you do.
You’ll need:
- A minimum of one year experience working with people with mental health needs, or people with multiple needs (e.g. mental health, homelessness, alcohol/drug dependency and offending).
- Proven experience of identifying risk, effective documentation and escalation of risk concerns.
- Strong communication and interpersonal skills.
- Ability to manage caseloads, work flexibly, and build effective multi-agency partnerships.
- Confidence in IT systems and record-keeping.
- A deep understanding of recovery and trauma-informed practice.
- A current, full driving licence and access to appropriate motorised vehicular transport (such as a car, moped or motorbike).
