HCRG Care Group Holdings Limited is seeking a Band 6 Registered Nurse/Paramedic to join our Hospital at Home service across Bath and North East Somerset, Swindon and Wiltshire (BSW).
You will work across community settings and patients' homes to deliver hospital-level care outside traditional hospital environments.
As part of a multidisciplinary team, you will assess, treat and manage patients who require acute care at home, supporting admission avoidance and early discharge pathways.
