Care Services Coordinator (Maternity Cover - Fixed Term 12 Months)

Salary: £26,790.40 (40 hours per week) + on-call payments (OTE £31,500 per annum)
We are looking for an experienced and compassionate Care Coordinator to join our team on a 12-month fixed-term maternity cover contract, starting in September. This is an exciting opportunity for someone who is passionate about building strong partnerships across health and social care to ensure people can return home safely and receive the support they need.
The primary focus of this role is to work closely with Great Western Hospital (GWH) and community healthcare professionals, coordinating referrals and supporting smooth, timely transitions from hospital to home. You will play a key role in developing and maintaining professional relationships with discharge teams, community services, and local healthcare partners, ensuring individuals receive responsive, person-centred care when they need it most.
You will work collaboratively with a wide range of professionals including Hospital Discharge Teams, Social Workers, Occupational Therapists, GPs, District Nurses, Community Healthcare Teams, and other partner agencies to coordinate care packages, respond to referrals, and promote positive outcomes for customers living independently at home.
Alongside this, you will support the day-to-day coordination of our domiciliary care service, helping to ensure high standards of care delivery and continuity for both customers and care staff.
Key Responsibilities
- Build and maintain excellent working relationships with Great Western Hospital, community healthcare teams, and local partner organisations.
- Coordinate referrals from hospitals, healthcare professionals, and community services to ensure safe and timely care at home.
- Liaise daily with Social Workers, Occupational Therapists, GPs, Hospital Discharge Teams, District Nurses, and other healthcare professionals.
- Support hospital discharges by arranging care packages quickly and efficiently.
- Ensure all referrals are managed professionally, with effective communication throughout the customer's journey.
- Promote collaborative working across health and social care services to achieve the best possible outcomes.
Care Coordination
- Develop and maintain efficient staff rotas that promote continuity of care and minimise travel time.
- Match carers with customers based on skills, experience, and individual needs.
- Manage last-minute changes, emergency cover, and staff absences.
- Act as a key point of contact for customers, families, and care staff.
- Respond promptly to telephone calls, emails, and messages.
- Record and elevate concerns, complaints, compliments, and safeguarding issues appropriately.
- Work closely with the Registered Manager and wider care team to maintain high standards of service quality.
- Participate in the shared out-of-hours on-call rota, providing support to customers and staff while managing emergencies and operational queries.
About You
We are looking for someone who has:
- A minimum of 1 year's hands-on care experience (essential).
- Experience working within domiciliary care, community services, hospital discharge, or care coordination (desirable).
- Excellent communication skills and the confidence to build professional relationships across health and social care.
- Strong organisational and problem-solving abilities.
- Experience in rota planning or scheduling (desirable).
- Confidence using care management or rostering systems (training provided if required).
- The ability to work independently while thriving as part of a supportive team.
