About the role
We are looking for an enthusiastic, skilled and motivated Stroke Specialist Nurse to join our newly established Early Supported Stroke Discharge (ESSD) team, delivering care across High Peak and North Dales. This is a rare opportunity to be part of the development of a new service, working in line with the established and highly regarded ESSD model used across Derbyshire. You will play a key role in enabling people to leave hospital earlier and recover safely and effectively in their own homes, delivering high-quality, evidence-based stroke rehabilitation.

This post is part of exciting county-wide work to improve the stroke pathway across Derbyshire. You will be joining something truly meaningful at a pivotal moment, helping to shape how stroke rehabilitation is delivered to our communities.
Key Responsibilities:
- To provide nursing expertise within the multi-disciplinary ESSD team, acting as an expert practitioner, liaising, guiding and advising the multidisciplinary team and external agencies in the provision of optimum nursing care for stroke patients on the ESSD pathway.
- To facilitate person-centred care, enabling people with stroke to manage their condition effectively and independently, preventing complications and improving quality of life.
- To participate in the development of effective, responsive, evidence-based and patient-centred services.
- To assist and/or lead on specific projects in agreement with the Team Leader and Operational Lead for the Service.
- To work in close collaboration with other teams providing stroke care, and with partners across health and social care in locality, to support the delivery of effective ESSD services.
