Coventry and Warwickshire Partnership NHS Trust is seeking a Flow Social Worker to join our Older Adults Service in the community.

The role focuses on maintaining patient flow and delivering strengths-based recovery, with at least 50% of time spent on Older Adult Wards.

You will work within a multidisciplinary team, receive clinical supervision, and contribute to service development while liaising with health, social care and voluntary partners.
