This is an exciting opportunity to work within a well-established multidisciplinary team supporting adults with learning disabilities across a range of community settings. The role focuses on promoting physical health and reducing health inequalities through high-quality, person-centred dietetic care.

The successful candidate will manage a specialist caseload and deliver dietetic assessment and interventions for individuals at risk of malnutrition, obesity, and diet-related health conditions. The role also includes opportunities to contribute to group work, health promotion initiatives, and service development.

You will be supported by a Highly Specialist Dietitian and work closely with colleagues across the wider CLDS team, including mental health, behavioural support, and transition services. The post holder will provide specialist dietetic input to adults with learning disabilities within Tower Hamlets, working across community settings including supported living, residential care, and service users’ homes.

Key responsibilities include:

  • Managing a defined caseload, providing dietetic assessment, intervention, and evaluation
  • Supporting individuals at risk of malnutrition through oral nutritional support
  • Delivering weight management interventions for individuals living with obesity and at risk of metabolic complications
  • Working collaboratively within the multidisciplinary team to support holistic care planning
  • Providing advice and training to carers, support staff, and other professionals
  • Delivering accessible health promotion initiatives, including group programmes
  • Supporting the development of easy-read resources and reasonable adjustments to improve accessibility

The role requires strong clinical reasoning, effective communication skills, and the ability to work autonomously within a community setting. Our mission is to make a positive difference to people's lives by improving the quality of life for all we serve. Our values are: We Care, We Respect, We are Inclusive – so we are looking for people who live and breathe these qualities when supporting service users and carers, and in their relationships with colleagues in the Trust and our partner organisations.

The Community Learning Disability Service in Tower Hamlets is a friendly and supportive multidisciplinary team committed to improving health outcomes for adults with learning disabilities. The team works closely with a wide range of partners, including primary care, acute services, and social care providers.

ELFT is a leader in Quality Improvement (QI), and staff are encouraged and supported to participate in service development, research, and innovation. You will have access to excellent professional development opportunities, regular supervision, and strong links with dietetic colleagues across the Trust, including specialist services such as Forensics and Eating Disorders.

We are committed to promoting equality, diversity, and inclusion and creating an environment where all staff feel valued and supported.

The post holder will:

  • Provide specialist dietetic assessment and intervention for adults with learning disabilities, using a person-centred and evidence-based approach
  • Manage a defined caseload with increasing autonomy, prioritising referrals based on clinical need and risk
  • Work collaboratively within the multidisciplinary team to contribute to assessments, care planning, and risk management
  • Deliver individual and group-based interventions, including weight management and nutritional support programmes
  • Identify and manage risks associated with malnutrition, obesity, and complex physical health conditions
  • Support individuals, families, and carers to access appropriate health services and make informed choices about nutrition
  • Develop and deliver accessible nutritional information and resources tailored to the needs of people with learning disabilities
  • Provide training and advice to MDT colleagues, carers, and support staff
