Allscreens Nationwide Ltd is seeking a PCN Clinical Pharmacist (Independent Prescriber) to work with SPINE PCN GPs. You will resolve day-to-day medicines issues, manage repeat prescribing authorisation, and perform medication reviews for patients, including those with multiple long-term conditions.
Some reviews may occur at patients' homes. You will support transitions from hospital to home, facilitate safe prescribing, and contribute to reducing waste and improving patient safety.
