Working Hours: 37 hours per week

Location: Observatory House/ Wexham Park Hospital (minimum 2 days per week in the office)

What you’ll be doing:

Are you an organised and proactive coordinator with a passion for improving outcomes for people leaving hospital? We are seeking an enthusiastic and highly motivated Business Coordinator to join our Hospital Hub team. This is a key role supporting the coordination of hospital discharge pathways and ensuring individuals receive the right support at the right time to enable safe and timely discharge from hospital.

Working closely with health and social care professionals, you will:

  • Manage referral processes
  • Coordinate assessments and service activity
  • Monitor performance data
  • Act as a central point of contact for partner organisations

You will play a vital role in supporting patient flow, reducing delays, and contributing to the delivery of high-quality integrated discharge services. The successful candidate will have excellent organisational and communication skills, strong attention to detail, and experience of managing data and administrative processes within a fast-paced environment. You will be confident working collaboratively with a range of stakeholders and committed to delivering excellent customer service.
