Are you an experienced Occupational Therapist looking for an opportunity to utilise and develop your clinical skills & leadership abilities? If so, our Specialist Occupational Therapy position may be perfect for you. The successful candidate will work within the Frailty team alongside other Occupational therapists and Physiotherapists. The team also includes a Consultant Geriatrician and specialist ACP's.

The team covers 'front door services' to include: Frailty Assessment Unit, the Emergency Department and Acute Floor (AMU, AAU, SDEC and short stay ward). These clinical areas are fast paced with a high turnover of patients, and would therefore require a dynamic and holistic individual. Our aim is to assess patients identified as frail on attendance or during admission, and use robust admission avoidance pathways to allow us to manage prompt discharge from the acute setting, in line with patient centred care and a home first model. The successful applicant will also support the weekend discharge/patient progression rota.

To assess, diagnose/interpret and manage own specialist caseload of complex patients as an autonomous practitioner in accordance with Professional Code of Conduct and Health and Care Professions Council Regulations. The post holder will work in close co-operation with colleagues within all Care Groups to deliver a cohesive service that is user led ensuring a consistently high standard of patient care, optimal patient flow that is safe and timely.

To work closely with team managers and senior therapists to ensure a robust structure of training, supervision and appraisal is delivered to all members within the team. To act as a source of Specialist advice and support within the Therapy Centre and across the Trust and the wider health service. Engage or lead on projects to develop services and learning opportunities for others including band 5's, assistant practitioners, support staff and students as well as celebrating learning and achievements with the wider therapy and MDT teams.

As a member of the Therapy Services Team your contribution to the profession of Occupational Therapy will be integral in enabling patients to receive holistic assessment to enable them to recover and be discharged from the 'front door'.
