Shrewsbury and Telford Hospital NHS Trust is seeking a Diabetes Nurse to promote the health and wellbeing of people living with diabetes, and to encourage adherence to medication through informed self-management. The post holder will collaborate with the Practice GP diabetic leads and nursing team to manage patients and develop the diabetes service.

You will identify clinical risks and take appropriate action, providing a diabetes care pathway and referrals to secondary care, multidisciplinary.
