Liaison Psychiatry Mental Health Practitioner

Able to work independently in a number of environments as an autonomous practitioner.

Provide comprehensive specialist mental health assessment and risk assessment of a wide range of service users.

Identification of referred patients' mental health needs, formulating and implementing nursing care plans and goals which will reflect the needs of the patients over a 24-hour period.

Regularly review caseload management with their clinical supervisor Maintain accurate records about the care and treatment offered.

Review and evaluate professional standards of profession in conjunction with the Senior Leadership Team.

Ensure that the developing requirements of the Mental Health Practitioner profession are understood by the wider team.

Ensure that clinical, supportive and therapeutic interventions protect the rights of the patient and are in accordance with evidence-based practice and Trust policies.

Adhere to confidentiality principles and maintain the rights of individual patients.

Contribute personal and professional skills, therapeutic use of self and those of other staff under their supervision, to ensure the development of the Mental Health Team Identify and manage risks in variety of settings.

Offering support and advice to families and carers and acting as an advocate for clients, where appropriate.

Manage a caseload by the effective use of time and resources.

Provide timely triage of service users in A&E and across the Teaching Hospitals.

Provide constructive supervision in clinical managerial and educational needs in relation to staff development and risk Management.

Influence/make and implement changes to clinical practice, locally and trust wide.

Participate in individual appraisal performance reviews and the construction of personal developments plan.
