The role involves working with the Programme Manager on the design, delivery, and evaluation of targeted initiatives that promote physical, mental, and social wellbeing across Pembrokeshire. This role is pivotal to the Economic Inactivity Trailblazer, focusing on developing one-to-one and group-based programmes designed to improve individuals’ ability to manage their health and reduce its impact on their capacity to work.

You will provide strength-based support, organising and ensuring the delivery of structured interventions, identifying the needs, aspirations, and challenges of residents to foster sustainable improvements. Working with those experiencing social isolation, poor physical health, or mental health challenges, you will help build the resilience and motivation required to move closer to employment or signpost them to suitable support for their needs. You will be working mostly in areas of greatest disadvantage in Pembrokeshire.

This role involves supervising a small team of Pathways Advisers, providing them with the toolkits, resource packs, and clinical oversight needed to embed health management into their daily coaching.

Key responsibilities

  • Design & Deliver - lead the development, coordination, and delivery of tailored community programmes, workshops, and wellbeing events, ensuring activities are inclusive, outcome-focused, and responsive to local need. Oversee programme quality, adapting content to help individuals overcome barriers to personal development, training, and progression.
  • Supervise & Support - provide day-to-day supervision, guidance, and project guidance to a small team of Pathways Advisers. This includes supporting the setting of clear objectives, offering regular one-to-one support, facilitating team development sessions, and ensuring consistent standards in the delivery of interventions. Support staff to build confidence and capability in conducting robust health and wellbeing assessments, while promoting reflective practice and continuous professional development.
  • Collaborate - build and maintain strong working relationships with a wide range of stakeholders, including GPs, healthcare professionals, and local organisations. Coordinate multi-agency approaches to ensure participants can access holistic, joined-up support pathways that address health, wellbeing, and employability needs.
  • Evaluate - oversee the monitoring and evaluation of programme impact, ensuring accurate data collection and meaningful use of both qualitative and quantitative feedback. Use insights to inform service improvements, support team learning, and contribute to strategic reporting and recommendations for senior leaders.
  • Champion - act as an ambassador for the Trailblazer project across Pembrokeshire communities, promoting awareness of integrated services. Lead by example in embedding equality, diversity, and inclusion across all activities, ensuring services are accessible and responsive to those facing the greatest barriers.
