We are seeking a highly motivated and experienced mental health professional to join our Integrated Recovery Hub (IRH) in Wandsworth as a Senior Practitioner / Clinical Team Manager. This is an exciting opportunity to play a key leadership role within a forward-thinking, integrated community mental health service, supporting adults with complex needs to achieve recovery and independence.

You will combine clinical expertise, leadership, and system-wide working to improve outcomes, reduce hospital admissions, and strengthen pathways between primary care, secondary care, and community services. The post holder will work as a senior clinical practitioner, Deputy Team Manager, and Practice Development Nurse within the Integrated Recovery Hub (IRH), supporting delivery of high quality, recovery-focused community mental health care for adults with moderate to severe mental illness.

The role combines:

  • Clinical leadership and caseload management
  • Operational support to the Team Manager
  • Practice development, quality improvement, and staff development

The post holder will:

  • Contribute to clinical and operational leadership of the IRH team
  • Support safe, effective flow through IRH pathways (psychosis, non-psychosis, complex emotional needs)
  • Lead on practice development, training, and implementation of evidence-based care
  • Promote multi-agency and integrated working in line with the IRH model

Location: Team is split between Springfield Hospital / Queen Mary's Hospital.

Clinical Responsibilities:

  • Provide highly specialist assessment, care planning, intervention and review for individuals with complex mental health presentations.
  • Hold and manage a complex clinical caseload, including patients within standard and intensive/complex pathways.
  • Ensure care aligns with NICE guidance, recovery-focused care planning, and trauma-informed approaches.
  • Lead on risk assessment, zoning, and crisis planning, ensuring safe management of risk.
  • Provide clinical consultation and expertise to MDT colleagues.

Leadership & Deputy Team Manager Responsibilities:

  • Act as Deputy to the Team Manager, supporting clinical oversight and decision making, and caseload and workforce management.
  • Provide clinical leadership within MDT forums, including MDT meetings.

Lead on practice development across the IRH team, ensuring delivery of evidence-based care. Design and deliver training programmes aligned to risk management and safeguarding, engagement and recovery approaches.
