The postholder will work as part of a community nursing service working across the Fylde coast, overseeing/supervising the provision of nursing interventions in patients’ homes or clinic settings. To manage a caseload of patients with highly complex needs using evidence base and patient centred principles to assess, plan, implement and evaluate interventions.
