We have an exciting opportunity with the Community Nursing team. We have opportunities throughout the Fylde coast that will be discussed at interview. We have Full & Part Time positions available as well as Permanent & Fixed Term.
Interested applicants will need to be registered nurses who have relevant nursing qualifications to degree level, specialist post registration education and leadership training or equivalent experience. The post holder requires at least 18 months' experience at Band 5.

We are looking for the successful candidate to be able to:
- Assess, plan, implement and evaluate care for a defined district nursing caseload.
- Ensure that the care needs of the caseload are met by effective management and appropriate delegation of work to other members of the team.
- Work in partnership with the primary health care team, liaise and cooperate with everyone involved in holistic patient care.
- Demonstrate extensive knowledge, advanced analytical and practical skills, and the ability to communicate in a non-judgemental, empathetic way with patients, carers and other professionals.
- Provide clinical supervision to junior staff and support staff development.
The post holder will need to have good communication skills both written and verbal, have evidence of ongoing professional development and be able to work independently.

The postholder will work as part of a community nursing service working across the Fylde coast, undertaking nursing duties in patients’ homes or clinic settings.
To provide safe, effective, individualised care to patients and to give support to families and service users requiring scheduled and unscheduled care interventions.
To monitor the condition and health needs of patients on a continual basis in partnership with colleagues, families and carers, referring to others for reassessment when required.
Hold a delegated 24 hr continuing responsibility for a caseload, co-ordinating admissions and discharges for patients registered to specific GP surgeries. Provide clinical leadership at weekends and bank holidays for staff within the Fylde Coast community Nursing Division on a rota basis.
Assess patients, plan, implement, evaluate and review the care of patients in their own home or a clinic setting. Establish and maintain associated clinical records.
Administer drugs in line with NMC Policy, including controlled drugs. If qualified in prescribing, prescribe in line with Trust guidelines, prescribing protocols and Nurse Prescribing Formulary.
Produce appropriate written reports when requested. To report and assist in the investigation of patient complaints.
Provide leadership and operational management of a community nursing team, supporting, supervising and managing staff.
To attend Team Briefing and Staff Meetings and disseminate the information and implement changes when appropriate within the team.
To delegate work to members of the Nursing Team, taking into account Scope of Professional Practice and nursing skill mix.
Liaise with other services within health and social care, voluntary and private sector to ensure a collaborative approach to service provision.
Contribute to the development of nursing services within the division.
Organise community schedules for students ensuring a robust educational placement is provided.
Provide an induction training package and clinical supervision opportunities for Preceptees and new staff.
