Restormel Community Mental Health Team is looking for a motivated and experienced Mental Health Nurse to join our friendly and supportive team within Cornwall Partnership NHS Foundation Trust. This is an excellent opportunity to make a real difference to the lives of people experiencing mental health difficulties and to support their recovery through high-quality, person-centred care.
We are seeking a dynamic practitioner to lead our duty desk service. The role involves managing incoming telephone calls, triaging referrals, coordinating information and working closely with colleagues across the multidisciplinary team. You will need to be highly organised, able to prioritise effectively in a busy environment, and confident in making clinical decisions.

This is a community-based role that may require frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.
You will be responsible for the assessment of care needs and clinical management of a defined caseload within the Cornwall Partnership NHS Foundation Trust (CFT) Mental Health Service. You will carry continuing responsibility for the development, implementation and evaluation of programmes of care, utilising the care co-ordination system. You will develop the interface between primary and secondary care services so the patients/carers are offered focused and co-ordinated packages of care that are appropriate to their needs.
