An exciting opportunity is available for an experienced Community Children’s Nurse to work in the Continuing Care Team as a clinical lead. This is a clinical role to manage the continuing care caseload, separate from the undertaking of continuing care assessments and care package set up, with a focus on leading areas related to direct clinical care.

Ensuring the Continuing Care Team (0-18 years) provides a high quality, evidence-based locally appropriate service to families by:

  • Identifying new theories, policies and guidelines relating to complex health needs care in the community;
  • Disseminating research findings and developing local policies and standards accordingly;
  • Providing professional and clinical leadership to all children’s community nursing staff, ensuring engagement with and adherence to local policies, guidelines and expectations;
  • Assessing nursing care needs, developing, implementing and evaluating programmes of nursing care for children and young people on the caseload.

The role will be integrated within the existing Community Children’s Nursing Team and the Children’s Continuing Care Team, part of an integrated network of services for children and young people in Bexley and Greenwich providing high quality specialist child-centred care. The team of specialist and generic children’s community nurses provides skilled, high quality, comprehensive nursing care to children and families with nursing needs in their own homes or community settings.

The post holder will be responsible for contributing towards ongoing developments of clinical practice, research and standards of care within the service, including policies, protocols, guidelines, and collaborating with the wider multidisciplinary team to implement acute pathways between the hospitals, GP and diagnostic hubs into the community.

The post holder will provide a high standard of evidence-based nursing care for children and young people with conditions such as disabilities and complex conditions, including those requiring palliative and end of life care. They will provide clear visible leadership, supervision, caseload management, and professional clinical expertise to the clinical skills nurse trainers in the team, ensuring every aspect of the child and family's experience is delivered in a safe, efficient and evidence-based manner.
