The Acute Care Support Worker will be expected to work as part of the Acute Pathway Team under the direction of the Service Manager. They will facilitate the smooth running of the section 136 Suite along with supporting the Access Team and ward areas in line with the Trust standards and policies.

The post holder will liaise and communicate effectively with relatives, community colleagues, other allied health professionals, external agencies, and medical/nursing staff. They will be expected to maintain confidentiality and adopt a positive attitude towards staff, patients, external agency staff and relatives at all times.

  • Liaise with the Police, Approved Mental Health Practitioners to organise Section 136 assessments.
  • Support the patient in the Section 136 suite whilst the Mental Health Act Assessment takes place.
  • Liaise with Police and senior staff regarding Section 136 advice.
  • Provide written documentation in the patient records, which details progression to discharge.
  • Answer the phone and deal with requests for information, referrals into the Access Team and support to people in distress, whilst maintaining a professional manner and ensuring confidentiality.
  • Communicate effectively with relatives/carers.
  • Relay timely messages to other members of staff and escalate any risk issues.
  • Build effective relationships with other Allied Health Professionals to ensure seamless transitions of care.
  • Undertake work that will help facilitate discharge of patients from the ward.
  • Identify beds for patients requiring admission.
  • Assist the relevant professional during assessment, treatment and intervention where required.
