Supporting Older Adults within the Trafford community, who are experiencing a range of complex mental health issues.
We work closely with families, carers and other health and social care professionals to promote recovery and autonomy.
We are a warm and friendly Multi-Disciplinary Mental Health Team with a strong focus on staff Health and wellbeing.
To support staff wellbeing, we offer flexible working as per the trust policy.
Within the team we have monthly CPD sessions in additional to regular Line Management and Clinical Supervision, as well as peer support.
We promote reflective practice and encourage practitioner development.

The post holder will
- Fulfil the role of Care Coordinator and the requirements of this role.
- Support a caseload of older adults with mental health problems, using evidence-based, client-centred principles to assess, plan, implement, evaluate, and record interventions.
- Provide support and advice to carers.
- Participate in the duty rota.
- Provide a nursing perspective within the multi-disciplinary team.
- Maintain nursing standards and contribute to the development of nursing practice in theteam.
- Contribute within the wider MDT in the development of clear care pathways regardingsupport, care and treatment of older adults with mental health problems.
- Work weekends and bank holidays on a rota basis, typically one weekend every 12 weeks.
- Participate in own personal development.
