The Support Worker will assist in implementing care and treatment plans, contribute observations to assessments and reviews, and maintain accurate clinical records using electronic patient record systems. They will work collaboratively with service users, families, and carers, providing information, practical support, and signposting to relevant services where appropriate. The post holder will work as part of a multidisciplinary team alongside Community Psychiatric Nurses, Occupational Therapists, Psychiatrists, Social Workers, Dementia Specialists, and Primary Care Services. Effective verbal and written communication skills are essential, particularly when supporting individuals with dementia, sensory impairments, communication difficulties, or mental health conditions. Working within community settings, the post holder must be able to undertake home visits, travel across the locality, manage their workload effectively, and use IT systems competently. Knowledge of older adult mental health conditions, safeguarding, the Mental Capacity Act, and person-centred, recovery-focused practice is required. The role demands compassion, resilience, professionalism, organisational skills, and the ability to work both independently and as part of a team.
