About the Role

As a Care Coordinator, you’ll be the key link between clients, families, and care workers. Reporting to the Branch Manager, you’ll ensure care is delivered smoothly, safely, and consistently — while supporting and developing our frontline teams.

Your Key Responsibilities

  • Create and manage staff rotas, ensuring appropriate travel time and breaks
  • Match carers to clients based on skills, preferences, and location
  • Manage new referrals from private clients and social services
  • Support assessments, care planning, and care package reviews
  • Communicate with clients, families, and healthcare professionals
  • Carry out risk assessments, reviews, and care worker supervisions
  • Ensure care documentation and client records are accurate and up to date
  • Support weekly KPIs, logbook drops, and team meetings
  • Organise and participate in supervisions, appraisals, and training
  • Take part in the on-call rota, including alternate weekends
  • Provide occasional support to neighbouring branches when required
