The Role
As a full-time Band 7 therapist specialising in frailty, you will work with doctors, advanced practitioners, nurses, therapists, and pharmacists to support frail patients both in the hospital and in the community, promoting a home first approach to the service.

Exciting Opportunity! Join the Frailty Team as a Specialist Therapist. You’ll play a key role in delivering the frailty service, ensuring timely, appropriate care is provided in patients’ own homes. You’ll work closely with patients and carers to manage health needs, promote self-care, and develop personalised plans. Collaborating with multidisciplinary teams, you’ll help shape high-quality, patient-centered services aligned with trust goals.
