Central London Community Healthcare NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting! This is a Band 6 role, and we would also welcome applications from experienced Band 5 clinicians with a strong background in Respiratory care who are looking to develop their skills within a supportive service.

You will work as part of a multidisciplinary community respiratory team delivering the Community Respiratory Service across West Hertfordshire. The service provides specialist respiratory care, including:

  • Hospital at Home (admission avoidance and early supported discharge, including Virtual Hospital)
  • Home oxygen assessment and review
  • Community clinics and home visits delivering expert care for patients with OSA, COPD, Asthma, Bronchiectasis and ILD

The role offers the opportunity to work autonomously while being well supported by the wider MDT. You will deliver specialist, patient centred care in a variety of settings, including patients' homes and nurse or physiotherapist led clinics, with a strong focus on improving outcomes and enhancing quality patient care.

As this is a community-based role, with travel between patient homes, there is a requirement to hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

The successful post holder will deliver patient centred, integrated respiratory care as part of a seven day service, supporting admission avoidance and contributing to the development of the Integrated Airways Virtual Hospital in partnership with the Hospitals Trust.

You will develop and apply specialist skills in the management of patients with respiratory conditions including COPD, Asthma, Bronchiectasis, OSA, and ILD, as well as supporting patients who require home oxygen therapy. This patient centred role includes:

  • Undertaking comprehensive respiratory assessments for patients with complex needs and co morbidities, including physical examination, clinical analysis, and the development of individualised, evidence based management and treatment plans.
  • Implementing specialist care programmes such as home oxygen therapy, pulmonary rehabilitation, medication optimisation, and psychological support interventions, in line with national and local guidelines.
  • Providing compassionate emotional support to patients and their families or carers living with long term and life limiting respiratory conditions.
  • Delivering care through a combination of community home visits and nurse or physiotherapist led clinics across West Hertfordshire.

As part of a large, experienced community respiratory service, you will be supported by a skilled multidisciplinary team including Band 7 and Band 8a clinicians, a Consultant Respiratory Physiotherapist, and Respiratory Consultants.
