To provide high-quality primary healthcare to low secure mental health patients, working as part of a multidisciplinary team as a primary healthcare Nurse. To lead on the delivery of high-quality physical healthcare within a Low Secure Forensic Unit, ensuring patients receive comprehensive assessment, monitoring, treatment, and health promotion interventions. The post holder will work collaboratively with the multidisciplinary team (MDT) to address physical health needs alongside mental health recovery, reduce health inequalities, improve health outcomes, and support patients to achieve and maintain optimum physical wellbeing within a secure care environment.

This role requires a NMC Registered General Nurse to work as part of a team delivering primary healthcare within a low secure setting. The physical health Nurse works within a Low Secure Forensic Unit providing specialist physical healthcare to adults with complex mental health needs who require care within a secure setting. The role involves:

• Undertaking physical health assessments
• Monitoring and managing long-term health conditions
• Coordinating physical health screening programmes
• Supporting health promotion initiatives
• Contributing to recovery-focused care planning

Working as part of a multidisciplinary team, the post holder liaises with psychiatrists, nursing colleagues, psychologists, occupational therapists, pharmacists, primary care services, acute healthcare providers, and other relevant professionals to ensure patients receive holistic and integrated care. The role requires the ability to balance physical healthcare priorities with the challenges associated with forensic mental health services, promoting patient engagement, supporting informed decision-making, and ensuring compliance with national standards, Trust policies, and evidence-based practice.

The post holder also contributes to staff education, service improvement initiatives, clinical effectiveness, and the continuous development of physical healthcare provision within the forensic pathway.

Responsible for the assessment, planning, implementation, and evaluation of healthcare for patients. Safe administration of medication in accordance with National, Trust and Local policies. Provide a triage service and clinical interventions, recognising environmental limitations and the need for appropriate referral, assessment, and transfer of patients who require care within wider NHS provision. Respond to healthcare emergencies within the hospital in accordance with the Trust's and Local policies and procedures. Deliver evidence-based care and seek specialist advice from other Healthcare professionals both internal and external. Handle sensitive and confidential information appropriately.

Organise and be responsible for the allocation of workload to deliver effective and efficient high-quality care. To provide basic leadership to junior staff by maintaining professional standards ensuring adherence to all relevant policies and procedures to ensure the delivery of high-quality care. Promote independent self-management of patients' healthcare needs. Respect patients' individuality and maintain their privacy and dignity within the constraints of the low secure environment. Actively participate in the development of the healthcare services within the secure hospital setting. Ensure the effective and efficient use of resources within their own sphere of responsibility. Prioritise own workload based on clinical priority. Responsible for identifying own learning needs and participate in training to develop and maintain own knowledge, personal qualities, and skills.

To actively participate in all aspects of clinical and managerial supervision. Take responsibility for your own and others' health and safety in the working environment. To work within the provision of and keep up to date with the Trust and Local policies and procedures. To undertake any other duties that would be a reasonable expectation of the role. Able to always recognise and maintain appropriate professional boundaries and report inappropriate behaviour/boundary concerns. Maintain own registration requirements in accordance with professional body regulation. Provide mentorship and preceptorship to junior staff and learners.

The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment. You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.
