The Community Sister/Charge Nurse for the Virtual Ward at Epsom and St Helier University Hospitals NHS Trust will work under the Virtual Ward Matron/Team Lead and collaborate with the East Surrey Hospital based Virtual Ward Frailty Team.
You will assess patients in the home setting, support early discharge from hospital and coordinate with social care and voluntary agencies to ensure safe care at home, while promoting staff development and evidence-based nursing practice.
