The predicted salary is between 37035 - 45265 Β£ per year.
Working independently and alongside GPs to identify appropriate care plans and treatment methods for patients through face to face appointments, telephone consultations and home visits.
Using clinical nursing practice at an advanced level to deliver quality care and advice to patients and demonstrate an in-depth knowledge of the options for achieving control of symptoms and assessing the health needs of patients with undifferentiated and undiagnosed problems.
The post holder will liaise with the care home to carry out a weekly ward round.
Prescribe medication appropriate to patient needs or refer the patient to a community pharmacy who will provide designated minor ailment services and prescription advice.
Provide leadership to the Practice Nurse and HCA. Support the nurse lead.
Supporting the practice and PCN delivery of the Covid response / Vaccine delivery.
Acting on Path Lab results / investigation report and hospital correspondences.
Administrative and Medication tasks.
