Service Manager (Proactive Neighbourhood Care) - Primary Care

Lead mobilisation from recruitment and induction through to go-live and business as usual.

Manage the care coordinator, HCA, bank clinical workforce and driver bank, with clear supervision and accountability.

Coordinate the shared workforce across proactive home visiting and remote monitoring to avoid duplication and improve value for money.

Ensure referrals, triage, visits, monitoring, escalation and discharge processes are safe, clear and consistently followed.

Work with Clinical Leads, GPs, paramedics, Local Authority teams, practices and partner organisations to resolve issues quickly.

Monitor activity, utilisation, patient outcomes, workforce capacity, quality, incidents, complaints and budget performance.

Produce accurate reports and dashboards and take action where delivery is off plan.

Ensure SOPs, risk assessments, information governance, safeguarding and CQC requirements are embedded.

Support continuous improvement and the ongoing development of the neighbourhood model.
