The integration team supports the development of integration and neighbourhood working in health and social care across Buckinghamshire, reporting into the Buckinghamshire Executive Partnership. The post holder will enable, promote and support the effective use of data, information, knowledge and technology to improve, inform and support a portfolio of projects and initiatives. They will support the management team to drive, implement and keep on track service developments.
They will work with integration leads to present performance data and progress updates to be presented accessibly in order to inform service and workforce planning and reporting to key stakeholders. They will also work with integration leads in engagement and communications of integration and neighbourhood work including with the patients and public.
The main duties of this role involve supporting the delivery of integrated neighbourhood working through effective project coordination, data analysis, and communication. The post holder is responsible for monitoring project plans, ensuring actions are completed on time, and assisting with service improvement initiatives. They play a key role in collecting and analysing performance data, presenting information clearly to inform decision‑making and identify risks.
In addition, the role supports operational planning, helping to implement service developments aligned with organisational priorities. The post holder also facilitates communication and engagement with patients, staff, and stakeholders, ensuring accurate and timely information is shared. Administrative duties such as organising meetings, taking minutes, and maintaining documentation are essential to keep projects on track. Overall, the role ensures efficient coordination and continuous improvement of services to support high-quality, patient-centred care.
