Are you looking for a varied and rewarding role working with people, supporting them to retain or regain independence following a hospital stay of period of illness, where you can make a difference in people’s lives every day?

If so, we are looking to recruit a Service Coordinator to join our existing Living Well Support Service Team Falls Prevention Winter Project, working across the Central Lancashire area.

The Living Well Support Service supports people to regain and maintain independence, helping to prevent hospital admissions and reduce the need for residential care.

In the last year, the service supported over 9,000 people across Lancashire.

This winter, we are expanding the service with targeted support for older people at risk of falls, loss of independence, and avoidable hospital admissions, helping them stay safe and well at home during the winter months.

The Falls Prevention Winter Project aims to keep older people out of hospital and reduce the frequent use of urgent and emergency care (UEC) services throughout winter, when vulnerable adults are at increased risk, by providing targeted interventions and support.

As a Service Coordinator (SC) you will

  • You will coordinate the delivery of a targeted falls prevention and frailty pathway for older people in a specific area.
  • You will manage referrals, oversee assessment activity, and develop effective relationships with health, social care and community partners.
  • Manage a team of Wellbeing Workers, scheduling and overseeing their work, providing support as required, and ensure service delivery targets are achieved.
  • You will use a solutions focused approach to ensure that people can access the help that they need when they need it, either from Age UK Lancashire or from other community-based support.
  • As the SC you will manage a small team of Wellbeing Workers who undertake holistic assessments to identify risks relating to falls, frailty, wellbeing, independence, social isolation and home safety.
  • You’ll ensure short-term in-home support is delivered safely and effectively through development of coordinated pathways and partnership working.

This role is predominantly office based, your work will include time spent within local hospitals, in the community, and at our Age UK Lancashire offices.
