Are you a highly experienced, enthusiastic, organised and motivated speech & language therapist with a specialist interest in stroke and neuro? We are looking for an experienced speech & language therapist with advanced clinical skills to be part of the established Community Neurological Rehabilitation Team (CNRT) in Central Lancashire. The CNRT provides ongoing rehabilitation to patients within the Chorley, South Ribble and Greater Preston locality with predominantly Stroke and ABI diagnosis.
You will have a commitment to working within an interdisciplinary environment, with experience of working with patients with stroke and neurological conditions. You will be offered regular supervision by a highly experienced therapist to support you in developing and expanding your existing skills and knowledge.
You will be responsible for:
- Assessment, treatment planning and management of rehabilitation programmes for a wide variety of clinical presentations.
- Delegation to generic rehabilitation assistants and supporting them in the delivery of this rehabilitation.
- Managing your caseload and diary.
- Regular in-service training as well as support for internal and external development within the field of Neurological Rehabilitation and leadership.
- Developing close partnership working with inpatient colleagues and other professionals.
Due to the nature of the role, it is essential that you can travel independently across the locality.
Your responsibilities will include:
- Assessment, treatment, planning and delivery of evidence-based rehabilitation interventions.
- Working as part of the rehabilitation and/or stroke/neurological service line and managing a clinical caseload of patients with complex needs.
- Comprehensive assessment of patients with complex presentations, using investigative and analytical skills to formulate an appropriate hypothesis and determine a diagnosis and management plan.
- Establishing individualised management and treatment plans, using clinical reasoning and a wide range of evidence-based treatment skills.
- Promoting health and reducing inequalities for all patients accessing the service.
- Contributing to clinical leadership and supervision, facilitating a culture of learning and reflective practice.
- Documenting and evidencing patient outcomes.
- Leading/contributing to training and development programmes as appropriate.
