Clinical care
- Assess undifferentiated patients through focused history, examination and observations; formulate and document a safe management plan.
- Recognise deterioration and red flags, using tools such as NEWS2 where appropriate, and elevate promptly to the most suitable service.
- Provide evidence-based treatment, advice, follow-up and safety-netting within agreed competence and prescribing authority.
- Verify expected deaths where trained, competent and authorised under local policy.
Frailty and neighbourhood care
- Identify frail or complex patients who may benefit from proactive support and contribute to holistic assessment and anticipatory care planning.
- Coordinate person-centred care across the three Downs Way Medical Practice sites and with PCN, community, care-home, social-care and voluntary-sector colleagues to support independence and avoid unnecessary admission.
Professional practice
- Work within HCPC standards, Downs Way Medical Practice and PCN policies and agreed competencies, recognising limits and seeking support appropriately.
- Prioritise a varied workload safely and maintain clear, contemporaneous records, confidentiality and information governance.
- Maintain training and CPD and contribute to safeguarding, audit, quality improvement and team learning.
