Age Connects Torfaen is looking for a Carers Officer to join our team and coordinate our short-break service for unpaid carers across Torfaen and Blaenau Gwent.

This is an exciting opportunity for someone who is passionate about making a real difference to carers' lives. You will be the Single Point of Access for short-break requests, working directly with carers to understand their individual circumstances, identify their needs and coordinate flexible, meaningful opportunities for them to take a break from their caring role.

About the role

As Carers Officer, you will:

  • Coordinate short-break opportunities for unpaid carers across Torfaen and Blaenau Gwent.
  • Meet and engage with carers to understand their needs, circumstances and preferences.
  • Assess carers' needs and identify appropriate short-break opportunities.
  • Coordinate and schedule short breaks with staff, volunteers and partner organisations.
  • Build positive relationships with carers, ensuring they feel listened to, valued and supported.
  • Follow up with carers after their short break to gather feedback and understand the impact of the service.
  • Identify additional support needs and provide appropriate information, advice and signposting.
  • Promote the service through community groups, health and social care services, GP practices and other local networks.
  • Develop strong partnerships with statutory, voluntary and community organisations.
  • Maintain accurate records and use CharityLog to record referrals, assessments and short-break provision.
  • Contribute to monitoring, evaluation, reports and case studies demonstrating the impact of the service.
  • Work flexibly across Torfaen and Blaenau Gwent, including community settings and home visits where appropriate.
