An exciting opportunity has arisen for a Band 6 Triage Nurse/AHP to join the Operations Hub Intermediate Care Triage Team. This pivotal role supports system flow, admission avoidance and timely discharge pathways through the delivery of high-quality, patient-centred triage. You will act as the point of contact for hospital discharge referrals and urgent community referrals, using your clinical judgement to direct individuals to the most appropriate Intermediate Care pathway to ensure people receive the right care, first time.

Working within a virtual MDT with system partners, you will apply sound clinical judgement to direct individuals to the most appropriate Intermediate Care pathway. We are looking for someone who demonstrates compassion, respect and collaboration, acts with integrity and accountability, and is committed to continual improvement in patient outcomes and service delivery.   • Comprehensive Triage of all referrals for the three Intermediate Care Services both hospital discharges and admission avoidance
  • Timely, consistent triage aligned to agreed pathways criteria

  • Allocate the Pathway 2 IPR beds, keeping a live and an anticipated capacity status to provide a proactive approach to discharge planning.
  • Signpost teams and external services appropriately.
  • With the support of the multidisciplinary team, hold meaningful, person-centred discussions to triage patients onto the most appropriate pathway.
  • Using specialist knowledge of all the discharge to assess pathways contribute to system wide multi-disciplinary case discussion to jointly agree the correct service based on the patients’ needs.
  • Work as part of a team, both within the Operations Hub and as part of the wider system.
  • Manage SystmOne caseloads and waiting lists effectively.
  • Work within service criteria to triage all patients referred in for an ICT Service.
  • Supervisor responsibilities for non-clinical team members, including completing monthly supervisions and supporting annual appraisals.
  • Maintaining up to date and accurate systems of processes.
  • Collate data manually, that cannot be extracted from clinical systems, in line with service data sets.
  • Provide “live” updates to referring teams on the status of all referrals using OPTICA or via email.
  • Record accurate notes relating to clinical triage within patient clinical record (SystmOne).
