We are looking for Integrated Support Workers who will travel across our local community, providing vital care and support in patients' own homes – helping people maintain their independence and live well where they feel most comfortable.

Our Home First and Virtual Ward models are at the heart of this transformation—enabling people to recover in their own homes with hospital-level support, surrounded by comfort, dignity, and expert care. This means:

You will be working independently but as part of a team, traveling to patients/clients' homes to deliver care. You should have experience delivering care to patients/clients (nursing or social care setting) and you will have a good standard of education which includes Maths and English GCSE (or the international equivalent).
