Paediatric Senior Nurse - PATCH

• 1. Clinical Responsibilities
• 1.1 Ensure high standards of patient care at all times challenging and ensuring others challenge poor practise, in accordance with the NMC Professional Code of Conduct.
• 1.2 In collaboration with senior members of the Divisional team provide assurance that the standards set out in the relevant Nursing & Midwifery strategy and direction of Nursing & Midwifery for the Trust are delivered.
• 1.3 Ensure local plans support all clinical quality external accreditation, for example Care Quality Commission (CQC) essential standards, Safety Thermometer, Commissioning for Quality and Innovation (CQUIN), Quality Accounts, NHS Litigation Authority.
• 1.4 Ensure agreed systems and process to consistently provide safe, effective high quality patient care and embed a culture of improvement, focusing on the Trusts harm free indicators, measuring practice and using variations in the data to develop local improvement projects are in place.
• 1.5 Through visible clinical leadership ensure that the delivery of kind, compassionate and respectful care takes place, and that the Project team develop relationships with patients that involve better listening and decision making; no decision without me
• 1.6 Act as the patients advocate when appropriate, when informed decisions may lead to choices being made concerning treatment options.
• 1.7 Ensure that the delivery of care meets the individual needs of all patients and families.
• 1.8 Ensure that all patient care is assessed, planned, prioritised and high quality, both physical and psychological delegating to support staff as required.

Seek advice and support from other staff as required.

• 1.9 In accordance with the individualised patient care, ensure clinical assessments are taken, results recorded and any appropriate actions taken based on the results.
• 1.10 Ensure the maintenance of professional contemporaneous records at all times, document all actions and interventions in a clear and honest and timely manner in line with Trust policy.
• 1.11 Develop effective teamwork, to ensure patients are regularly reviewed and discussed as part of regular MDT meetings, to include ongoing care plans especially for nights, weekend and bank holiday periods.
• 1.12 Ensure nursing team monitors patient progress, working towards safe and timely intervention.
• 1.13 Using professional judgement be prepared to intercede and act as patient advocate to clarify prescribed plans of care.
• 1.14 Deal with concerns raised by patients and visitors in a proactive manner, review patient feedback results and update the Projects patient experience improvement plans, as needed.
• 1.15 Champion a safeguarding culture, which meets the needs of all vulnerable patient groups.
• 1.16 Ensure relatives and other staff are engaged in a professional manner and the team act in a courteous and polite manner at all times, thereby ensuring good public relations between patients, public and staff.
• 1.17 Store, check and administer drugs in accordance with the Trust Medicines policy.
• 1.18 Work off site when required with the possibility of conducting home visits as a sols worker
• 2. Management Responsibilities
• 2.1 Ensure nursing practices comply with Trust procedural documents.
• 2.2 To have local open and transparent communication channels that support the Project team to do the right thing for people we care for, to be bold when they have good ideas and to speak up when things go wrong.
• 2.3 To ensure the Trusts risk management policy is appropriately utilised.
• 2.4 Manage and lead the clinical team when on duty with overall continuing clinical responsibility, in order to carry out delivery of care to a cohort of patients both in the hospital and community.
• 2.5 Manage the delegated budget for the Project, working closely with the Lead Consultant, and Finance managers, in order to deliver the project brief within the confines of the allocated funds
• 2.6 Develop the Project clinical team, ensuring a learning environment, leading by example and to be aware of the effect of change upon all staff.
• 2.7 Ensure the health and safety of all staff, patients and visitors, reporting any incidents, hazards, or defects and taking responsibility to minimise risk.

To implement action plans to ensure improvement if an incident has occurred.

At all times adhering to and ensuring adherence to Trust policies and procedures.

• 2.8 Identify and lead on changes to the working model of the Project as it develops. Communicate changes to all relevant clinical and managerial staff involved in the project.
• 2.9 In accordance with Trusts policy for Provision of Safe Staffing and Skill Mix levels, formally agree with Divisional Director of Nursing the Projects workforce requirements, utilising acuity and dependency tools monitor and review of nursing workforce as indicated, ensuring integration with the local delivery plan, and the optimal deployment of the nursing workforce.
• 2.10 In line with the Trusts e-Rostering policy deploy staff to ensure safe and appropriate nursing care is given at all times.

This includes the approval of off-duties anticipating problems where they exist and taking local / timely actions as appropriate.

• 2.11 Take responsibility for human resource issues including appraisal, personal development, sickness absence, disciplinary and grievance, recruitment and selection and personal career development.
• 2.12 Take an active role in planning care and ensure close and effective liaison with other agencies following a patients discharge.

Liaise with community support services to ensure satisfactory levels of service and where concerns bring these to the attention of the Project lead Consultant

• 2.13 Ensure all complaints both formal and informal are dealt with promptly and courteously in accordance with Trust policy.

To respond in writing to complaints by interviewing staff, collating information and writing statements to provide evidence, reviewing practise where appropriate in response.

• 2.14 Actively participate in local and Trust level meetings proactively seeking to improve patients experience and feeding back relevant information to the Team.
• 2.15 Meet regularly with the Lead Consultant to monitor clinical care, finance/resources and HR issues, ensuring clear communication pathways into and out of the department.
• 2.16 Handle, check and store patients cash and valuables in accordance with the Trust policy.
• 2.17 Assist the lead Consultant in producing Project progress reports on measurable outcomes.
