- 1. Clinical Duties
- Undertakecomprehensive physiotherapy assessments, including detailed history, physical examination, outcome measures, and risk assessments.
- Manage acomplex, diverse clinical caseloadindependently, prioritising appropriately and modifying management as needed.
- Formulate and deliverindividualised evidence-based treatment plans, using advanced clinical reasoning.
- Provide specialist interventions such as: Manual therapy
- Exercise rehabilitation
- Self-management planning
- Falls prevention strategies
- Long-term condition management
- Respiratory physiotherapy (if part of local PPHS scope)
- Behaviour change and lifestyle advice
- Participate in and leadmultidisciplinary team (MDT) work, contributing to integrated care plans and joint reviews.
- Support early identification of deterioration and contribute toacute-avoidance pathwayswhere applicable.
- Ensure all clinical documentation is completed to HCPC, CSP, and Trust standards.

- Deliver physiotherapy that aligns with PPHS principles of: Early intervention
- Providing group-based and 1:1 rehabilitation programmes where needed (e. g., falls, strength & balance, prehabilitation).
- Work collaboratively with other PPHS professionals to streamline referral pathways and enhance patient flow.
- Contribute to health promotion strategies such as physical activity, pain management education, and lifestyle modification.
- Participate in outreach programmes including home visits, community clinics, and virtual consultations.
- Provideclinical supervision, mentoring, and supportfor Band 5 physiotherapists, students, assistants and other junior staff.
- Assist in the induction and training of new staff members.
- Lead or supportin-service training sessions, case discussions and CPD events.
- Act as a role model for professional behaviour, high clinical standards, and compassionate practice.
- 4. Service Development & Quality Improvement
