Sanctuary Trust is looking for a caring and motivated Community Connector to support people who are homeless, at risk of homelessness, or struggling to keep their housing. You will help people find the right support, build confidence, and become more independent. This is a rewarding role where you can make a real difference in your local community.
What you'll do:
- Build positive, trusting relationships with individuals who are rough sleeping, vulnerably housed, at risk of homelessness, or requiring support to sustain a tenancy.
- Act as a consistent point of contact, helping individuals access housing, wellbeing, financial, employment, volunteering and community-based services.
- Signpost and accompany individuals to relevant services, programmes and activities where required.
- Deliver early intervention and tenancy sustainment support to prevent homelessness and reduce escalation into crisis.
- Facilitate access to peer support, encouraging participation from people with lived experience in shaping support pathways and community activities.
- Support individuals to build confidence, independence, social connections and a sense of belonging within their local community.
- Promote strengths-based approaches that develop individual skills, aspirations, life skills and financial independence.
- Onboard volunteers/peer mentors providing inductions and supervisory support.
- Support the delivery of Sanctuary Trust's Smart Sharing model, to enhance tenancy sustainment programme and resident wellbeing.
- Help individuals achieve personal goals relating to housing stability, wellbeing and social inclusion, including the development of supportive peer networks.
- Support individuals accessing the Solidarity Lockers and Managed Postal Address services, promoting safe use and engagement with wider support.
- Provide flexible, trauma-informed support to people with complex needs, including those requiring gradual engagement and individuals at risk of exploitation.
- Develop and maintain effective partnerships with voluntary, community, faith, statutory and homelessness organisations, contributing to multi-agency approaches to homelessness prevention.
- Maintain accurate records, support plans, case notes and outcome data, contributing to project monitoring and reporting requirements.
- Ensure compliance with safeguarding, confidentiality and data protection requirements, maintaining professional boundaries and promoting the safety, wellbeing and dignity of all individuals accessing support.
- Help maintain the integrity of the Environmental and Quality Management Systems according to standards ISO14001 and ISO9001.
- Engage in required training and CPD to stay updated with administrative practices, policies, and regulatory compliance.
- Any other duties as directed by Senior Management.
