Join our Brent Urgent Community Response (UCR) Team as an Occupational Therapist and make a meaningful difference to people’s lives. You will deliver expert, home-based care that supports individuals to remain independent, recover safely and avoid unnecessary hospital admission. As a valued member of our multidisciplinary team, you will provide expert occupational therapy assessments and interventions in people's homes, supporting recovery, promoting independence and enabling individuals to remain within their communities whenever possible. Working closely with colleagues across health and social care, you will contribute to a dynamic service that responds quickly to changing needs and makes a real difference to people's lives every day.

The post holder will provide high-quality occupational therapy assessment, intervention and rehabilitation to patients across a range of community settings, including care homes, residential units and individuals’ own homes. You will work closely within a multidisciplinary team to support admission avoidance, facilitate early hospital discharge and promote independence and recovery.

Key responsibilities include:

  • Undertaking holistic assessments and delivering evidence-based interventions tailored to individual needs.
  • Prescribing, ordering and providing education on the safe use of equipment and adaptations.
  • Contributing to effective care planning and risk management to support safe care at home.
  • Supervising, mentoring and supporting the development of junior staff and students.
  • Building strong, collaborative relationships with patients, families, carers, colleagues and partner agencies.

You will be expected to work flexibly and contribute to the delivery of a responsive, patient-centred service that meets the needs of the local population.
