The Urgent Community Response Service (UCRS) at ESNEFT is seeking a highly motivated and skilled nurse to join our dynamic and innovative team.
This is an exciting opportunity to deliver high-quality urgent care in patients' homes and care home settings, helping people receive the right care at the right time while avoiding unnecessary hospital admissions.
Working within our multidisciplinary team, you will use your advanced clinical skills to assess, diagnose, prescribe, treat and manage patients with a wide range of urgent healthcare needs.
No two days are the same, making this an ideal role for an experienced clinician who thrives on autonomy, clinical decision-making and providing outstanding patient-centred care.
As a Nurse Practitioner within UCRS, you will
- Undertake rapid response assessments within two hours of referral.
- Assess, diagnose and manage urgent and complex clinical presentations in community settings.
- Independently prescribe appropriate treatments and interventions.
- Deliver short-term, intensive support to prevent hospital admission and facilitate recovery at home.
- Manage patients with frailty, multiple long-term conditions, acute deterioration and palliative or end-of-life care needs.
- Provide expert clinical advice and support to patients, carers and healthcare professionals.
- Work collaboratively with GPs, therapists, community teams, social care and acute services.
- Contribute to service development, quality improvement and clinical leadership within the team.
