The Specialist Community Public Health Nurse (SCPHN) leads the delivery of the Healthy Child Programme within a 0-19 integrated public health team in Nottingham. You will manage an assigned caseload, signpost families to other services, and work with the Early Help team to deliver evidence-based packages of care to support children, young people and families.

You will support the Clinical Service Leads and Managers to deliver an integrated service offer, and collaborate with partners to ensure.
