Community Stroke Rehabilitation Support Worker | Nottinghamshire Healthcare NHS Foundation Trust

We have an exciting opportunity for a part time Stroke Community Rehabilitation Support Worker to join our established Integrated Community Stroke Service to deliver rehabilitation into various community settings. The role of Stroke Community Rehabilitation Support Worker in the Multidisciplinary Community Stroke Team would be to provide high quality holistic, evidence based care to patients / service users that is responsive, effective and efficient. You would work with Occupational Therapists, Physiotherapists, Speech and Language therapists, Clinical Psychologist, Assistant Practitioners and Rehab Support Workers.

Rehabilitation is currently delivered 8am-5pm, 5 days a week, including Bank Holidays. Weekend working may be required in the future. This role requires day-to-day delivery of therapeutic care in our Integrated Community Stroke Multidisciplinary Team of AHP’s. You will need to:

• Demonstrate your passion and clinical ability to assess patients who present with significant physical health care needs.
• Support our patients within the community to enable them to live their optimum life, being self-caring and independent with their health needs following a stroke.
• Support to deliver therapy treatment plans, working collaboratively within a multidisciplinary framework, providing compassionate, realistic and timely support to a diverse patient group and their families.
• Demonstrate excellent communication and interpersonal skills with the ability to be adaptable, reliable, resourceful and approachable, evidencing high levels of personal and clinical credibility, being able to establish effective working relationships in a range of work settings.

As a Community Stroke service, we aim to offer a high quality therapy led Multidisciplinary Team approach providing stroke rehabilitation in a patient’s home focused around achievement of agreed rehabilitation goals. As a Rehabilitation Support Worker in the Community Stroke team you would be expected to deliver therapy treatment plans with stroke clients under direct and indirect support of the qualified clinicians.

The Community Stroke Team is split into three localities. The base of this role is within the locality of Nottingham West. The team work closely and will require you to work flexibly, across the whole of South Nottinghamshire, to support our evolving and developing service, supporting the team to deliver high quality integrated care.

This position requires you to work independently, but also be a team player, working closely with our Integrated Care Provider partners providing care to a diverse population covering many different geographies including, Broxtowe, Gedling and Rushcliffe.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment. You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies. A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
