We are looking for a part-time Stroke Community Rehabilitation Support Worker to join our dedicated Community Stroke Team working across the South of Nottinghamshire. This role is 0.6wte (3 days per week). We are looking for someone who can demonstrate motivation, drive, commitment, and passion for promoting health, well-being, and independence for stroke patients in the community.
As a Community Stroke service, we aim to offer a high-quality therapy-led Multidisciplinary Team approach providing stroke rehabilitation in a patient's home focused around the achievement of agreed rehabilitation goals. As a Rehabilitation Support Worker in the Community Stroke team, you would be expected to deliver therapy treatment plans with stroke clients under the direct and indirect support of the qualified clinicians.

We are an experienced existing provider of Community Stroke Rehabilitation and as part of our service, we provide a review for patients after stroke at 6 and 12 months delivered by our Stroke Reviewing officers. With a strong focus on quality improvement, we continually strive to make a difference, providing an integrated and person-centred approach enabling transformation of services to ensure outstanding delivery of care to the people and communities we serve.

We therefore seek a compassionate, dynamic, innovative Therapy Assistant Practitioner to join us on this journey and ensure our patients continue to receive exceptional care.
The Community Stroke Team is split into three localities. The base of this role is within the locality of Nottingham West. The team works closely and will require you to work flexibly across the whole of South Nottinghamshire, supporting the team to deliver high-quality integrated care.
This position requires you to work independently but also be a team player, working closely with our Integrated Care Provider partners providing care to a diverse population covering many different geographies including Broxtowe, Gedling, and Rushcliffe.
As you settle in, you will receive regular, meaningful clinical and managerial supervisions and an appraisal within 4 months of commencing in your role to establish how we can support you to be the best version of yourself, whilst achieving your developmental needs and aspirations.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment. You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.
A full UK driving licence and vehicle for business use is required for this post; however, reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
Please note that this post does not meet the pay or skill level required for a Skilled worker visa. Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. This salary/skill level is below the minimum required for sponsorship for a Skilled Worker/Health & Care visa.
